We are currently aware of a performance issue affecting CareDirector V5 – Connecting Care.
Initial investigations have identified a significant security change made earlier this morning, which involved amendments to multiple user accounts. Details of this change have been shared with the national team for review and further clarification.
We are continuing to monitor the situation closely and will provide updates as more information becomes available.

Our monitoring indicates that the system is now recovering, and we expect performance to return to its standard operational level shortly.
We will continue to observe system behaviour to ensure stability is maintained and will provide a further update if required.
Resolved
System performance has now stabilised and has remained at the expected operational level for a sustained period. As such, we are marking this incident as resolved.
The root cause was identified as a set of security changes made outside of the agreed change window. These changes were applied during the morning period, which is when system throughput is at its highest and the platform is most sensitive to performance-impacting activity.
Following an earlier update confirming that performance had returned to expected levels, we are aware of a further degradation that began at approximately 10:30.
Our monitoring shows a clear correlation between this new spike in system latency and a recent security-related change applied shortly beforehand. This aligns with the root cause of the previous performance issues.
We are actively investigating this recurrence with our technical teams and will provide a further update as soon as we have more information.

Following our previous update, our monitoring now indicates that the CareDirector V5 platform has returned to its expected operational performance levels.
Based on our investigation, we believe the root cause of the performance issues was related to recent security changes, which led to elevated query times on several key procedures. These have since stabilised, and no further degradation has been observed.
We will continue to monitor the system closely to ensure performance remains consistent, but at this stage no further action is required.

We have seen a general improvement in system performance; however, it has not yet returned to normal operating levels across all areas. Our investigations have identified several queries that were contributing to the slowdown, particularly during the time of the reported issues.
In particular, we observed elevated query times on the following stored procedures:
- p_cascaderevokeaccess
- p_cascadegrantaccess
- p_systemuserbuentitymapreinit
These procedures relate to user security and permissions, and the delays appear to be linked to recent changes in this area.
Our technical teams are continuing to investigate to restore full performance as quickly as possible. We will provide a further updates shortly.
